Insurer Won’t Provide CPT Codes

My health insurance company refuses to provide me with the CPT code.  They will confirm the CPT code if I give it to them.  But they will not provide it to me.  Given that I have an 80/20 plan and I want to practice good accounting principle this seems consumer unfriendly and relying on them to a fault –given their history of errors.  Any thoughts about this policy.  I do not think it is industry standard.

How can I get the CPT codes I need?
